Q&As Notice

Question: I have been notified that my client has passed away. What are my documentation obligations?

Answer: After documenting in the client’s record the date and individual providing the notification, retention of clinical records remains the same for clients who are living and deceased. A record is retained for at least 10 years from the date of the last entry in the record or the date 10 years after the day on which the client reached, or would have become, 18 years old as outlined in the Standards for Record Keeping.
