In a number of situations, occupational therapists, their employers, facility operators offering privileges to them, and other regulated health care providers have a legal obligation to make a mandatory report.

Mandatory reports are not optional. Mandatory reports must be made in writing, by email, surface mail or fax, or by completing the online form below. 

There may also be times where you become aware that an occupational therapist may not be practising safely or behaving professionally. Depending on the circumstances, it may be appropriate to bring this to the attention to the occupational therapist themself, their direct supervisor, the employer, or, in some cases, make a report to the College.

Reports cannot be anonymous.
